Please submit all changes to somer.sizemore@usosm.com,
amanda.ramirez@usosm.com and copy

Date of Updates:     s/9/25

Updates Submitted B; : Dena Rathbun

Practice Name and Office Location: Alamo Maxillofacial Surgical Associates TX04- All Doctors

Request Details:

The new Medical Center location is not credentialed for Medicare Part B (all medicare MEDICAL plans and MEDICAL replacement/advantage plans). As a result, Medicare for MEDICAL patients can only be seen in Stone Oak and Boerne. Dr. Torres remains opted out for Medicare as well.

Until this issue is fully resolved, please follow the following guidelines for scheduling MEDICARE MEDICAL patients.

  1. Dr. Buckley, Franco, Verrett, Hanna and Rominger can be scheduled for ONE Medicare
(MEDICAL) patient per day in Stone Oak and Boerne.
  2. NO Medicare MEDICAL patients can be scheduled in Medical Center until further notice.
  3. Dr. Torres remains ONN for Medicare MEDICAL patients.
  4. If there is a need for more than one Medicare patient for a doctor on a given day, please seek approval from PCC.
  5. This change DOES NOT apply to DENTAL medicare plans (Dental coverages that come along with Medicare replacement or advantage plans). The dental plans are not affected.
  6. Identifying MEDICAL in nature appointments is important to avoid unknowingly scheduling Medicare medical patients in the new Medical Center location. Here are red flags that would indicate a patient may be MEDICARE (if they don't know).
Patient is OVER 65, is coming in for TMJ, pathology, trauma or pre-radiation extractions. Please avoid scheuduling these patients at Med Ctr.

Is this update temporary or permanent? Permanent

It’ temporal)', please provide details on duration: Until further notice
